How it Works
You choose a dentist from the DentaCap network and there are no claims forms to file or deductibles to meet. You must stay within the network in order to be reimbursed for covered services.
Services provided at NO charge include:
- dental examinations
- x-rays
- prophylaxis (cleaning; 2/yr.)
Orthodontics:
- The plan covers orthodontic services for you, your spouse and covered dependents up to 19 years old
- There is a $1,000.00 savings
