Children may be registered in a District 59 school only if their parents/guardians/legal custodians are legal residents of School District 59. Separate residency rules apply to homeless students and students eligible for special education services.
Parents should bring their child’s certified and numbered birth certificate or passport, multiple proofs of residency and emergency contact information when registering for school. Proof of residency must include one of the following: most recent real estate tax bill, signed lease, closing statement for the purchase of a residence or notarized letter from the owner of the residence. A second proof of residency should include two of the following items: drivers license, utility bill, insurance certificate, public aid card, or voter registration card.
All children are required by the State of Illinois School Code to furnish documented evidence of physical examination and proof of immunity against Diphtheria, Pertussis, Tetanus, Poliomyelitis, Measles, Mumps, Rubella and Chickenpox within one year prior to entering school for the first time (pre-kindergarten, kindergarten or first grade), and again in sixth grade. Children who are able to show proof of having previously had the chickenpox need not receive the chickenpox immunization. The required form must be returned to the school office on or before the first day of class. Any child(ren) not in compliance with this health requirement will not be permitted to attend school. The only exceptions are those that are religious in nature or if a particular vaccine is medically unsafe for the child. In those cases, proper documented evidence must be submitted to the school nurse in order to give credence to the exception. New students in the district are given a 30-day period to comply with this requirement.
Children entering school for the first time or upon entering fifth, sixth, seventh or eighth grade must have received at least the first in the three-shot series of Hepatitis immunizations. A doctor’s note stating when they are to receive the second and third shots must also be provided to the school Health Office. Students who have not received the first shot may be excluded from school.
All students entering 6th, 7th, 8th, 9th, 10th, 11th, 12th will be required to supply proof they have received one dose of Tdap (defined as tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTaP, DT or Td dose.
In accordance with state law, all students in grades kindergarten, second, and sixth grade must provide proof of dental examination by a licensed dentist by May 15th, 2013. The dentist must complete the form entitled “Dental Examination Record”, which is available at registration sites or on the list below.
All children enrolling in Kindergarten, and any new student enrolling in a school for the first time, must provide proof of having an eye examination within the previous year by October 15th. Eye examination forms are available at registration sites or on the list below.
When registering your child(ren) you may also want to bring with you the following:
- Transfer and copy of health records from previous school, if it applies
- If new (copy of immunizations)
- Emergency contact names and phone numbers (3 if possible)
- Current home and work phone numbers, including cell phones email addresses
