All field trips must be scheduled between 9:00 AM and 1:55 PM unless prior arrangements have been approved by the Principal and the Transportation Department. Field Trip requests will not be processed if the information on the Request Form is incomplete or is not received within the required time frame.

Step 1: Bus Transportation Request

  • Teacher submits the Field Trip Request Form (T-40) to the Principal no less than 4 weeks prior to the trip. Trip requests will be processed in the order in which they are received. Be aware that buses may not be available as requested.

Step 2: Bus Transportation Approval

  • Principal/Secretary forwards request to Transportation Department (woodhouse.pam@ccsd59.org) no less than 3 weeks prior to the trip. Trip requests will be processed in the order in which they are received. Be aware that buses may not be available as requested.
  • Transportation Department confirms bus availability and schedules buses. School Assistant Secretary will be notified availability has been confirmed. School Assistant Secretary notifies teacher.

Step 3: Field Trip Permission Slips, Fee Collection, and Lunch Requests

  • No permission slips or fees are allowed to be distributed and/or collected until the field trip has been approved by the Principal and the Transportation Department. After approval, the teacher sends Field Trip Permission Slip (B-41) home with students. Permission must include any fee to be charged and the option for all parents to request a sack lunch for their child. Parents must respond 2 weeks prior to the field trip.
  • Teacher submits all monies collected to building secretary along with a list of attendees detailing who has paid. DO NOT keep cash or checks with you or in your classroom. All monies collected from students must be deposited by the school secretary. Students who qualify for fee waivers are not required to pay for field trips that are required or are a customary part of a class or extracurricular activity.
  • Secretary must submit a Check Request to cover the entry fees not less than 1 week prior to the trip. Check requests must be received by Accounts Payable on or before the Tuesday of the week prior to the field trip.
  • A Field Trip Lunch Request Form (SN-010) listing all students who have requested a sack lunch must be submitted by the school secretary to Sodexo not less than 1 week prior to the trip. Coolers will be provided to transport milk and are the responsibility of the teacher to distribute at the lunch site. Students who do not qualify for free meals (reduced price or full paying students) must have money in their lunch accounts in order to receive a sack lunch.

Step 4: Student Health Needs and Emergency Information

  • Nurse/health assistant needs to provide updated list of student health concerns and those requiring medication to be included in this request. Be certain student emergency contact information is readily available and accurate.
  • Confirm with your principal the processes and procedures to be followed in case of an emergency. Be sure the school office is fully informed of the teacher’s emergency contact phone number while on the field trip.
